Payment Terms

  • Please reach out to us with any questions and concerns. We appreciate working with you.

  • Payment terms for full payment are within 30 days from statement date. Unpaid balances over 30 days of statement date will incur monthly interest of 2% (24% annualized).

  • Unpaid balances over 30 days will require pre-payment for any new work requested until the account is in good standing.

  • Unpaid balances over 45 days will result in a hold on all work in process until the account is in good standing. If the balance is not paid within 15 days of the 45-day notice, the amount will be submitted to an external collection agency.

  • As always we accept check payment if this is your preference. Please note that returned checks or a Non-Sufficient Funds situation occurs we will charge any fees incurred by 38 Smiles from financial institutions.

  • We promise to make every effort to bring to your attention any outstanding balances and resolve any billing issues.

Payment Policy

  • An account is opened and secured by your credit card with cases charge as each case is shipped.

  • After 90 days, accounts in good standing may choose to accrue invoices into monthly statements generated on the first of the subsequent month.

  • This detailed statement will be mailed to you: Payment is due by the 11th.

  • 38 Smiles Dental Laboratory, Inc. accepts payment by all major credit cards.

  • For credit card payments, with your authorization, automatic payment will be charged to your card on the first of each month. A receipt of your transaction will then be mailed to you. Monthly statements will continue to be mailed on the first of each month. If you desire to participate in this option please call us at (657) 600-8971 and we will acquire the necessary permission.

Remake & Warranty Policy

  • The cost for fabricating custom-made dental appliances cannot be refunded.

  • A credit may be issued by 38 Smiles Dental directly to a customer’s account when cases do not meet 38 Smiles’ standing for quality, function and aesthetic. Any credit balance on an 38S account must be used/applied for lab services within 60 days from the date of issue or it will expire.

  • By sending a case to 38 Smiles Dental Laboratory you are agreeing to these terms of the Remake & Warranty Policy.

  • Remake Policy: Eligible remakes will be done at no charge of received within 60 days of invoice date. All original case materials including models and impressions along with the dental restoration MUST be returned for credit consideration.

  • New cases will be billed if remake is required due to any of the following:

  1. There is a shade or product change different from the original request. Including but not limited to, type of material, shade, design, etc.

  2. Lab questioned impressions, margin, or bite and was advised to complete case.

  3. Lab requested a try-in, customer declined and asked for a completed case.

  4. Abutment required reduction due to undercuts/clearance and/or the teeth were re-prepped.

  5. A new impression was sent for the item to be remade with.

  6. A patient’s allergy was not stated on a lab script prior to fabrication.

  7. A change in treatment plan was verbally given instead of properly submitted with a new laboratory prescription.

  • A canceled treatment plan by the patient or office does not warrant a return or credit of any kind and the lab is not involved with patient-to-provider plans.

  • Warranty is for 1 year from date of delivery on invoice.

  • This warranty is in lieu of all other warranties, weather expressed or implied and may not be modified by any agent, employee, representative or distributor of 38 Smiles Dental Laboratory.
